Be sure you have the latest point release installed. Unzip the contents
of 3dcondm1.zip to your Quake3\baseq3 folder. Start Quake 3 and choose
single player/skirmish, or multiplayer/create from the main menu. You should
be able to select 3dcondm1 from the screenshots toward the end of the list.
